ABSTRACT

Introduction: In chronic diseases, QoL depends on several factors such as the type and duration of the disease, its treatment and side effects, the severity of symptoms, medication effects, patient age, limitations and self-care capacity. In chronic kidney disease it has also become a measure of health outcome.Objective: assess the association between QoL and socioeconomic, lifestyle and clinical factors of patients on HD.Methods: This is a cross-sectional epidemiological census, carried out among 1,024 patients on hemodialysis from all hemodialysis units at the metropolitan region in the Espirito Santo's, Brazil . Sociodemographic, lifestyle and clinical characteristics data were used. QoL was assessed using the Short-Form health Survey-36 (SF-36). Data were analysed by multiple linear regression.Results: The best QoL was represented by mental health (72.16) and the worst QoL by the physical aspect (26.78). After multiple linear regression, predictors of QoL were identified, with emphasis on males, a predictor of better QoL for 5 of the 8 domains and the summaries of the physical and mental components. Education, associated with 5 domains, gives greater reference to income. Physical activity was a predictor of 7 of the 8 QoL domains and the summary of the physical component. Among the clinical variables, the number of complications is associated with 7 of the 8 domains and summaries of the physical and mental components.Conclusion: Male sex is highlighted as a predictor of better physical and mental health and elderly people with better mental health, despite the impairment in physical health. The absence of physical activity is associated with worse physical and mental health. Clinically, having 3 or more intradialytic complications was associated with worse physical and mental QoL

Objetivo: avaliar a associação entre qualidade de vida e estado nutricional de pacientes em hemodiálise, segundo medidas antropométricas e bioquímicas. Métodos: estudo transversal realizado com 1.024 pacientes de 11 centros de hemodiálise de uma região metropolitana da região sudeste do Brasil. Resultados: por meio da regressão linear múltipla, foram identificados os preditores nutricionais de cada domínio da qualidade de vida. Destacamos a albumina sérica, o Índice de Massa Corporal (IMC) e a circunferência da cintura, dentre as medidas associadas tanto a saúde física quanto a mental desta população. Conclusão: a inadequação do estado nutricional está associado a pior qualidade de vida física e mental de indivíduos em hemodiálise. Além do monitoramento do estado nutricional, a avaliação nutricional prediz a qualidade de vida e torna-se uma ferramenta fundamental para um melhor desfecho de saúde, uma vez que a baixa qualidade de vida é um dos principais problemas desta população

Métodos: estudio transversal realizado con 1.024 pacientes de 11 centros de hemodiálisis de una región metropolitana del sureste de Brasil. Resultados: mediante regresión lineal múltiple, se identificaron los predictores nutricionales de cada dominio de la calidad de vida. Destacamos la albúmina sérica, el índice de masa corporal (IMC) y la circunferencia de la cintura, entre las medidas asociadas a la salud tanto física como mental en esta población. Conclusión: el estado nutricional inadecuado se asocia con una peor calidad de vida física y mental en los individuos en hemodiálisis. Además de monitorear el estado nutricional, la evaluación nutricional predice la calidad de vida y se convierte en una herramienta fundamental para un mejor resultado de salud, ya que la baja calidad de vida es uno de los principales problemas en esta población

ABSTRACT

OBJECTIVE: To evaluate the correlation between thickness of the muscle adductor pollicis and anthropometric measurements, body mass index and Subjective Global Assessment in the nutritional assessment of surgical patients. METHODS: The study population comprised patients admitted to the general and reconstructive surgery unit of a university hospital in the city of Vitória (ES), Brazil. The inclusion criteria were patients evaluated in the first 48 hours of admission, aged ≥20 years, hemodynamically stable, with no edema or ascites. Data analysis was performed using the software Statistical Package for Social Science 21.0, significance level of 5%. RESULTS: The sample consisted of 150 patients that were candidates to surgery, mean age of 42.7±12.0 years. The most common reasons for hospitalization were surgical procedures, gastrintestinal diseases and neoplasm. Significant association was observed between thickness of adductor pollicis muscle and Subjective Global Assessment (p=0.021) and body mass index (p=0.008) for nutritional risk. Significant correlation was found between thickness of adductor pollicis muscle and arm muscle circumference, corrected arm muscle area, calf circumference and body mass index. There were no significant correlations between thickness of adductor pollicis muscle and triceps skinfold and age. CONCLUSION: The use of thickness of adductor pollicis muscle proved to be an efficient method to detect malnutrition in surgical patients and it should be added to the screening process of hospitalized patients, since it is easy to perform, inexpensive and noninvasive.

RESUMO Objetivo Avaliar a correlação entre a medida da espessura do músculo adutor do polegar com medidas antropométricas, índice de massa corporal e Avaliação Subjetiva Global no diagnóstico nutricional de pacientes cirúrgicos. Métodos Participaram do estudo pacientes internados em uma unidade de cirurgia geral e reparadora de um hospital universitário de Vitória (ES), Brasil. Os critérios de inclusão foram pacientes avaliados nas primeiras 48 horas de internação, idade ≥20 anos hemodinamicamente estáveis, sem edema ou ascite. A análise dos dados foi realizada pelo softwareStatistical Package for Social Science, versão 21.0, com nível de significância de 5%. Resultados Compuseram a amostra 150 pacientes candidatos à cirurgia, com média de idade de 42,7±12,0 anos. Os motivos de internação mais comuns foram procedimentos cirúrgicos, doenças do trato gastrointestinal e neoplasias. Observou-se associação significativa entre a espessura do músculo adutor do polegar com a Avaliação Subjetiva Global (p=0,021) e com o índice de massa corporal para risco nutricional (p=0,008). Foram encontradas correlações significativas entre a espessura do músculo adutor do polegar com circunferência muscular do braço, área muscular do braço corrigida, circunferência da panturrilha e índice de massa corporal. Não foram encontradas correlações significativas entre a espessura do músculo adutor do polegar com prega cutânea triciptal e idade. Conclusão O uso da espessura do músculo adutor do polegar mostrou-se um método eficiente na detecção de desnutrição em pacientes cirúrgicos e deve ser acrescentado ao processo de triagem de pacientes hospitalizados, uma vez que é de fácil execução, baixo custo e não invasivo.
